PERSONAL DATA APPLICATION PROCESS
Personal data subjects, defined as data subjects under Law No. 6698 on the Protection of Personal Data (“KVK Law”) (hereinafter referred to as “Applicants”), are granted the right to make certain requests regarding the processing of their personal data under Article 11 of the KVK Law. Pursuant to the first paragraph of Article 13 of the KVK Law, applications to our Company, the data controller, regarding these rights must be submitted to us in person, in writing in Turkish, and via the following methods determined by the Personal Data Protection Board (“Board”).

According to the Communiqué on the Procedures and Principles for Applications to the Data Controller (Official Gazette No. 30356 dated 10/03/2018), the following information must be included in applications.
Information to be Completed During Application:
• Name, surname, and signature if the application is in writing.
• Turkish Republic ID number for citizens of the Republic of Turkey; nationality, passport number, or ID number, if applicable, for foreigners.
• Residence or workplace address for notification purposes.
• Email address, telephone, and fax number, if applicable.
• Subject of the request.
Further information and documents related to the subject should be attached to the application.
Regarding your personal data If you wish to apply, the application form below must be completed in its entirety. This application form has been prepared to identify your relationship with our Company and, if applicable, fully identify your personal data processed by our Company, so that we can respond to your application accurately and within the legally required timeframe. To eliminate legal risks that may arise from unlawful and unfair data sharing and, in particular, to ensure the security of your personal data, our Company reserves the right to request additional documents and information (such as a copy of an ID card or driver's license) for identification and authorization purposes. If the information you submit regarding your requests is not accurate or up-to-date, or if an unauthorized application is submitted, our Company accepts no liability for requests arising from such inaccurate information or unauthorized application.
In accordance with Article 13, Paragraph 2 of the Personal Data Protection Law, your submitted applications will be responded to in writing, free of charge, within thirty days of the date your request is received, depending on the nature of the request. However, in cases where a written response is required, a fee of up to ten pages will be charged. A processing fee (1 TL) will be charged for each page exceeding ten pages.
